Parent and Student Education Option Selection Form

Dear Horizon Family, 

As we approach the end of the first semester, we are planning for the remainder of the 2020-2021 school year. With that being said, we are asking for your input.

For the remainder of the 2020-2021 academic year, Horizon students will be asked to choose from the following three learning options: 

1.Traditional Learning Option: Students will attend school five days a week. They will be responsible for completing any work assigned to them by their teachers. 

  1. Hybrid Learning Option:  This will allow students to return for in-person instruction two days a week while remaining six feet apart. The remaining three days will be spent at home doing online instruction. 

Group A would attend school Monday and Tuesday with at home instruction (online) Wednesday, Thursday, and Friday.

Group B would attend school Wednesday and Thursday, with at home instruction (online) Monday, Tuesday, and Friday.

  1. Remote Learning Option: In the event you are concerned about your family’s and/or child’s safety, we will offer a fully-remote learning option. This option will allow students to engage entirely with the curriculum (online) with support from their teachers. 

Please keep in mind that if there is a current “Stay-at-Home” advisory, ALL students will remain on remote learning. The safety of our students and staff is our first priority. 

Please complete the following form to make a selection for the remainder of the  2020-2021 academic year. 

Your selection must be made on or before Friday, December 18, 2020. Once a selection has been made, it cannot be changed until the end of the third quarter (March 12, 2021). 

Our plans for the upcoming year are designed with an ability to quickly pivot to remote learning. Should circumstances change and require all students and staff to switch to remote learning, we are prepared to do so.
